Scheduling an appointment with Green Relief Health is easy. You can either visit us at our convenient Baltimore location or you can get your medical cannabis certification from the comfort of home via a tele-health appointment.

Remember, when you make your appointment to be seen by a certified Maryland medical cannabis provider, you will have to fill out some forms for us to review your medical history to establish your qualifying condition.

Green Relief Health Medical Marijuana Appointment Checklist

To ensure a successful visit with our medical marijuana doctor, be sure to bring the following with you to your appointment:

Proof of Identification

Bring one of the following:

• Current and valid State Driver’s License
• Current and valid State ID card
• Current military issued ID card

OR

• An out of state ID or other photo AND
• Two Forms of proof of in-state residency such as utility bill, lease, bank statement, etc.

State of Maryland ID Number

This is the number you receive from the Maryland Medical Cannabis Commission we need to process your certification.

Intake Forms

These are Green Relief Health’s forms that we need to have to complete your visit and they include the following:

• Medical Release Form
• Intake Form

Appropriate Medical Documentation

Bring along any necessary medical documentation to support your diagnosis. This includes:

• Recent medical records
• List of current prescriptions (if any)
• Physician’s letter (optional)
• X-rays, MRI’s, or other test results (if applicable)

Payment Method

Credit card or money order for payment UNLESS you have paid in advance.

Other

• If you are under age 18, you must be accompanied by a parent or legal guardian
